Instructions for Completing ICAO Format FAA Flight Plan Form 7233-4
This document provides basic instructions for filling out the International Civil Aviation Organization’s (ICAO) formatted flight plan form, FAA Form 7233-4. Notice that the upper portion of the ICAO format flight plan is not to be filled out by the submitter, as this is for FAA use only. You will also see that there are numbers in front of some fields, and not in front of others. These numbers correlate with system numbers used by the FAA, and are referenced in AIM guidance. You will begin with entry field 7, Aircraft Identification.
Explanations for entries you will need to make in each of the fields are shown below. Where a more detailed explanation is needed, a link is provided to a reference document. When filling out this form, be sure to use CAPITAL LETTERS only.

WAKE TURBULENCE CAT. | L | Max. certified takeoff weight of 15,500 lbs. or less. |

CRUSING SPEED | (e.g. N0300) or (e.g. M053) | True airspeed in knots entered as “N” followed by 4 digits, or Mach cruise entered as “M” followed by 3 digits. |
LEVEL | (e.g. F280) or (e.g. A070) | Cruising altitude in Flight Level entered as “F” followed by 3 digits, or in Altitude entered as “A” followed by 3 digits. |

JACKETS | J and X | Enter J if life jackets are available, and then place an "X" in the features NOT part of the jackets. |
DINGHIES | X | Enter X's if NO life raft is available. If a dinghie is available, use 2-digits to indicate number, 3-digits to indicate capacity of all dinghies aboard, and enter an "X" if the dinghies are covered. |
AIRCRAFT COLOR AND MARKINGS | Alpha characters | Describe the colors and markings of the aircraft to aid in Search and Rescue operations. |
COLOR | (e.g. ORANGE) | Enter the color of the dinghies. |
REMARKS | Alphanumeric | X out the "N" box if no remarks are entered. This field is for remarking about additional survival equipment or features. |
PILOT-IN-COMMAND | Alpha characters | Enter the First and Last Name of the PIC |
